1. Complete the Athletics Participation Form
2. Complete the Health History and Concussion Form

Then email your Physical form to JYoung@capeelizabethschools.org

All of this needs to be approved prior to participation . The school nurse is likely working today but likely won’t be over the weekend so please do today!
