ALL REGISTRATION FORMS AND PAYMENT ARE DUE BY 8/17.

If this is not complete by the equipment handout on 8/17 we will not be able to issue equipment and your son will not be able to practice or participate in team functions until the registration is 100% complete. Attached are the forms and fee requirements from the high school athletic department. I have also attached the link to the website for those who need to fill out asthma or allergy action plans. Through this link you will also see the “Concussion Consent Form”, this needs to be filled out directly through the online portal.
